血友病合并艾滋病和慢性丙肝感染者haart 3年副作用的观察

    【摘要】  目的  观察血友病合并艾滋病和慢性丙肝感染者用高效抗逆转录病毒治疗（haart）3年的副作用。方法  对43例血友病合并艾滋病和慢性丙肝感染者的病人用高效抗逆转录病毒治疗3年。定期采用流式细胞仪检测cd4、cd3、cd8、nk计数; fda批准的病毒载量仪（bdna）检测血浆病毒载量进行随访。结果  43例病人在治疗3年后cd4+平均上升257/mm3（p＜0.001），hiv rna下降，治疗后平均2.26±1.10log/ml（p＜0.0001）。治疗后alt、cg、iv.c上升（p＜0.05）。结论  haart能快速抑制hiv rna的复制，但是对肝脏是有影响的。

    【abstract】  objective  the goal to observe the hemophilia patients with hiv/hcv co-infection using haart side effect within three years.methods  43 cases of hiv/hcv co-infection received the haart for three years. regularly we use flows type cell meter to examine cd4+, cd3+, cd8+, nk cell counts. the blood plasma virus load was tested by fda authorization viral load meter (bdna) and all patients were carried on during follow-up.results  of 43 patients after three years of treatment  cd4+ average counts rise to 257/mm3 (p＜0.001), hiv rna concentration decreased, after haart average value reduced to 2.26±1.10log/ml (p＜0.0001). but serum alt, cg and the iv-c level were risen (p＜0.05).conclusion  haart can rapidly suppress hiv rna replication, but it is influenced to the liver.

    自1994年实施高效抗逆转录病毒治疗(haart)以来，与人类免疫缺陷病毒(hiv)相关的各种感染性疾病的发病率和艾滋病(aids)病人的病死率大幅度下降的事实已是众所周知。

    人类免疫缺陷病毒(hiv)和丙型肝炎病毒(hcv)的传播途径相似，在静脉注射毒品者（ivdus），血友病病人和同性恋者人群中可以引起合并感染，并可导致病毒生物学的行为改变，使感染者临床表现复杂化，特别是因目前haart的广泛开展,治疗对肝脏副作用的影响逐渐引起大家的注意。
