The rule of the House of Durazzo was contested by the Dukes of Anjou of the House of Valois, who led several military expeditions into the kingdom. In the end Queen Joan II, being heirless, recognized Duke Louis III in 1426 as Duke of Calabria and heir. Louis predeceased her, but his brother René inherited his claim. Joan recognised René as her heir before her death.

Louis I, Duke of Anjou, was the adopted heir of Joan I. He succeeded her, de jure, on her death in 1382. His descendants fought the House of Durazzo, mostly in vain, but not without any successes, for the throne until an agreement was reached between Louis III and Joan II whereby she recognised him and his house as her heirs. René, Louis's brother, succeeded Joan in 1435.
René had a contestant in King Alfonso V of Aragon who had been previously considered as a successor by Joan II but had been later discarded in favour of René's brother. Alfonso conquered the kingdom manu militari and René was forced to flee. René's claim was inherited by either his nephew (Charles IV of Anjou, who died in 1481, leaving his claims to French king Louis XI) or his grandson (René II of Lorraine). The latter's descendants continued to claim the throne of Naples, as did the French kings, down to 1529, and intermittently until 1559.

Joachim Murat was the first king to rule a kingdom which was called "Two Sicilies" by the Edict of Bayonne, in 1808, though he controlled the mainland, he never physically controlled the island of Sicily which his Bourbon rival had fled from Naples to.After the Congress of Vienna, the title king of Two Sicilies was adopted by Ferdinand IV of Naples, in 1816.Under Ferdinand the Kingdom of Naples and the Kingdom of Sicily were unified, he had previously been king of both Naples and Sicily.

Roger II received royal investiture from Antipope Anacletus II in 1130 and recognition from Pope Innocent II in 1139. Manfred was regent of Sicily for his nephew, the child Conrad II ("Conradin"), but took the crown in 1258, and continued to fight to keep the kingdom under the Hohenstaufen. In 1254 the pope, having declared the kingdom a papal possession, offered the crown to the King of England's son, Edmund Crouchback, but the English never succeeded in taking the kingdom. In 1262 the pope reversed his previous decision and granted the kingdom to the King of France's brother, Charles of Anjou, who succeeded in dispossessing Manfred in 1266. Conradin continued his claim to the throne until his death by decapitation perpetrated by Charles of Anjou in 1268.

Peter III of Aragon, Manfred's son in law, of the House of Barcelona, conquered the island of Sicily from Charles I in 1282 and had himself crowned King of Sicily. Thereafter the old Kingdom of Sicily was centred on the mainland, with capital at Naples, and although informally called Kingdom of Naples it was still known formally as "Kingdom of Sicily". Thus, there were two "Sicilies" — the island kingdom, however, was often called "Sicily beyond the Lighthouse" or "Trinacria", by terms of a treaty between the two states.
