今天由来自华中科技大学同济医学院附属普爱医院(武汉市骨科医院)的方真华主任和肖凯医生为大家主讲「慢性踝关节外侧不稳定合并距骨骨软骨损伤的治疗」。
本文主要阐述慢性踝关节外侧不稳定与距骨骨软骨损伤的关系,分别介绍二者的治疗方法,分析慢性踝关节外侧不稳定与距骨骨软骨损伤的合并治疗及其效果。
● 踝关节外侧不稳定:由于外侧踝关节稳定结构的异常或损害,或/和神经反射及本体感觉功能的障碍,导致在生理负荷下无法控制踝关节活动,反复扭伤从而出现踝关节功能障碍的一种状况。
● 外踝稳定结构中,静力稳定结构包括距腓前韧带、距腓后韧带、跟腓韧带,动力稳定结构包括腓骨肌腱和支持带。
● 距腓前韧带等外侧韧带扭伤不一定会导致距下关节不稳定。
● 踝关节内外侧韧带损伤、距下关节损伤的前抽屉试验(ADT)是不同的。
外侧副韧带损伤导致距骨软骨损伤的机制如下:
● 内翻、旋后姿势会将最大压力作用于距骨内侧缘
● 外侧副韧带切除后会导致踝关节各个方向接触面积增加
● 外侧副韧带切除后应力显著性的增加在内翻和旋后位
● 距骨软骨损伤多位于其内侧缘
● 慢性踝关节外侧不稳定容易引起距骨骨软骨损伤
● 慢性踝关节外侧不稳定引起包括距骨在内的软骨损伤,即使外侧韧带手术重建非常成功,术后患者满意度仍然不显著。
● 慢性踝关节外侧不稳定是导致踝关节软骨病理改变的重要因素。
● 慢性踝关节外侧不稳定引起踝关节骨软骨损伤比例增加。
● 慢性踝关节外侧不稳定引起距骨内外侧软骨损伤,内侧损伤比例高于外侧软骨损伤
● 慢性踝关节外侧不稳增加了距骨软骨损伤的严重程度,损伤程度与踝关节不稳定的时间呈正相关。早期治疗能够增加患者满意度。
踝关节外侧副韧带的手术治疗手段包括手术修复、重建术等。
● 改良Broström术
● 骨膜翻转加强
● 腓骨肌腱转位修复
● 自体肌腱转位修复
● 恢复关节软骨光滑表面
● 解除患者症状,改善关节功能
● 生物力学的再平衡/正常透明软骨生物力学特性
● 阻止或延缓局部关节软骨损伤导致终末期骨关节炎的进程
目前的治疗策略包括微骨折、自体/异体骨软骨移植、自体软骨细胞移植(ACI)、组织工程概念、关节置换等。
由于篇幅限制,本文介绍微骨折、自体骨软骨移植两种方法。
微骨折虽然有效但并不是真正恢复软骨结构。
● 2年临床效应显示,骨软骨损伤面积越大微骨折处理临床预后越差
● 镜下探查修复软骨面毛糙
● 局部组织活检提示修复软骨为纤维软骨
● 修复的软骨样组织随时间的推移而逐渐退化

● 优势:自体软骨修复
● 不足:应用于小的缺损区受限,供区并发症,边缘修复困难,多柱填塞后保持软骨面平整困难
● 自体软骨移植长期随访(平均84个月)显示,患者踝关节功能及症状改善显著。MRI影像结果显示,软骨及软骨下骨修复。
踝关节外侧不稳定会导致软骨损伤,但二者合并发生率缺少大样本的数据支持,据报道分别为95%(Taga et al.)、25%(Komenda et al.)、66% (Hintermann et al.)。
● 手术方案:踝关节镜软骨微骨折、改良Broström外侧副韧带修复
● 处理距骨软骨损伤的同期修复外侧副韧带,手术效果更佳确定,临床功能恢复良好
● 距骨软骨损伤联合慢性踝关节外侧不稳手术指征明确,恢复踝关节稳定性是获得良好预后的前提;临床预后与软骨损伤面积相关,与软骨损伤深度相关性不大
● 手术方案:距腓前韧带修复;距骨软骨表面粗糙——清理,缺损小于15mm——微骨折,缺损大于15mm——自体移植
● 内侧或者外侧距骨骨软骨损伤(OLT)联合慢性踝关节外侧不稳(CAI)手术治疗临床预后差异较大。尽管ATFL重建修复满意,然而内侧OLT微骨折处理后期效果仍不显著。
● 因此,作者认为,对于踝关节不稳内侧软骨缺损治疗时,微骨折并不十分适合软骨损伤的治疗。

距骨骨软骨损伤联合外侧副韧带不稳定获取临床优良满意度的因素包括:
● 早期发现,早期诊断,早期治疗
● 恢复踝关节稳定性
● 同期合理处理软骨损伤
● 对于踝关节外侧不稳定引起的关节损伤的研究仍在继续,并不断深入
● 踝关节外侧不稳定能够引起距骨骨软骨损伤,证据充分
● 早期发现,早期同时治疗踝关节外侧不稳定及距骨骨软骨损伤是提高临床预后的关键
● 内侧距骨骨软骨损伤合并踝关节外侧不稳定的临床预后相对欠佳,选择合适的骨软骨损伤治疗方案显得异常重要,单纯微骨折可能并不十分合适
