一名全科医生的自白
在你印象中,全科医生的生活应该是什么样的呢?今天,就让我们走近月坛社区卫生服务中心的杨医生,让我们一起感受她的故事,感受全科医生的工作体验。

“第一次见到这位病人时,她大概五十多岁。除了‘愁眉苦脸’,我真的找不到别的词来形容她。她当时超重,始终愁眉苦脸,没有一丝一毫的笑意。她只会说‘麻烦给我按这个处方开下药’。她话真的很少。我就笑着说,‘大姐,我可以了解一下您的饮食和锻炼情况吗?’然后我收到的只是沉默。很尴尬,我也不知道怎么收场。这位大姐总是从我手里拎起药就头也不回地走了。”

“类似的状况,我会一遍又一遍地遇到。等到第十次,我又重复问起这个问题时,终于得到了回应。‘麻烦不要再问了。给我开药就成了。三级医院会给我诊断的。我的病治不好!’但我没放弃。最后终于有一天,她告诉了我她午餐和晚饭吃了什么,还说了自己夜晚心悸的症状。以我的理解,这些症状表明她低血糖,当前用药并不适合她。于是,我建议她换药。但她只留下一句话:‘不要向我推销新药;我知道你就想要提成。’”


“等到她下次再来开药,我就主要向她提供一些饮食和锻炼方面的建议。同时,我也简单提了一下不同糖尿病药物的机理。几次这样的对话之后,我又开始进一步谈我推荐给她的药物背后是什么机理,告诉她这种推荐的药为什么对她更有用。我成功说服了她试用这种新药。几天后,她又来到我办公室,我第一次看到她脸上有了笑容。‘杨医生,您好!我要感谢您给我推荐的药。开始吃这种新药后,夜里我慢慢不再心悸了。真的要谢谢您!’”

“那天之后,这个病人好像一切都变得明亮起来了。你可以看到,我的建议真正的效果,是她开始信任我,开始接受我对于她日常饮食和锻炼的建议。适当的药物、健康的膳食再加上有规律地锻炼,三者相互配合,她成功减掉了30斤!整个人气色也好了,笑得也更坦然了!每次她来见我,都会说:‘你知道吗,我觉得自己越来越年轻了!’”


“现在,她的血糖水平控制得很好,处于健康范围内;她自己也知道怎么自测血糖水平;知道自己的血糖水平恢复正常,她也很高兴!另外,我们因为经常交流,她对我来说就像一位朋友、一位家人。她知道我们在社区卫生服务中心上夜班很忙,知道我们有时候甚至没时间吃饭——所以她经常给我做饭,把热菜热饭送到我办公室来。这些菜都是我们经常吃的家常菜,让我觉得特别温暖、特别感动。”

“帮助像她这样的病人慢慢养成健康的生活方式,帮他们更好地进行自我健康管理,让我认识到自己全科医生工作的意义,很有成就感。结合他们自己的特殊情况,基于作为一个普通人的我和他们之间的关系,我不仅给他们开药,还给他们提有用的健康建议。还要说的是,在诊疗过程中,我可以和病人之间建立紧密的个人联系。我理解他们的感受,他们也理解我的想法。想到有些地方医患关系很紧张,再想到我的病人带给我的温暖,我真的很感恩。”

Hello patient, #HelloGP – Through the Eyes of the Doctor

Today we’d like to introduce you to Dr Yang, a GP at Yuetan Community Health Centre. Listen to her story about what it’s like to be a GP. 
“When I first met this particular patient she was a woman in her 50s - the only word I can use to describe her is ‘gloom’.  She was overweight. She frowned all the time. She never offered even the faintest smile. ‘Please give me the medicine on this prescription’, she would ask. She was very economical with her words. ‘Dear Madam’, I would smile to her and say, ’May I know your diet and exercise plans?’  Silence.  I did not know how to end the awkwardness. The lady would just grab her medicine from my hand and leave.

The same situation repeated time and time again. When I asked the same question for the tenth time, I finally got a response from the lady. ‘Please do not ask any more. Just give me the medicine. I will get a diagnosis from tertiary hospitals. My diseases cannot be cured.’ But I kept trying. Finally, one day, she told me what she ate for lunch and dinner, and her symptoms of heart palpitation at night.  To me, these symptoms indicated that she was suffering from low blood glucose, and thus the current medicine was not suitable for her. When I advised her to change her medicine, she just left me with one sentence, ‘Do not market new medicine to me; I know you just want your kickbacks.’ 


The next time this lady came in for her medicine, I focused more on providing advice around diet and exercise. At the same time, I talked a little about the mechanism of different medicines for diabetes. After several conversations like this, I began to talk more about the mechanism of the medicine I recommended for her, and how this medicine could work better for her. I managed to persuade the lady to try a new type of medicine. After several days the lady returned to my office, but this time with a smile on her face – for the first time. ‘Hi Doctor Yang,’ she said. ‘I want to thank you for your suggestion of the medicine. The suffering at night has gradually dissipated after I started taking the new medicine. Thank you so much.’

Everything seemed to become brighter for this patient after that day. You see, the real effect of my advice is that this lady came to trust me, and started to take my advice about her daily diet and exercise plans. With the coordination of suitable medicine, healthy diet and regular exercise, the lady succeeded in losing over 15kg! Moreover, she looks much better and smiles more readily. Every time she comes to me, she always says, ‘You know, I feel I am growing younger.’ 


Right now, the lady’s blood glucose level is well-controlled and in the healthy range, she is knows how to test her own blood glucose levels, and she is very happy knowing that her blood glucose level is now normal.  Furthermore, through our interaction she has become like a friend or a family member to me now. She knows we get so busy on the night shift here at the Community Health Centre, that we sometimes do not even have time to have dinner – so she often cooks for me and sends the hot dishes to our office! These dishes are just what people usually eat at home, which makes me feel so warmly appreciated.

Helping patients like this lady to develop healthy lifestyles over time, and take better charge of managing her own health, makes me realize and appreciate the meaning of my job as a GP.  I do not only offer them medicines, but also useful health advice – according to their own specific conditions and based on my relationship with them as a person. What is more, during the treatment process, I can establish a tight personal connection with my patients. I understand their feelings, and I think it is true the other way around. Given the doctor-patient relationship can sometimes be tense; I really appreciate the warmth my patients bring to me.”
