后踝撞击综合征（Posterior ankle impingement syndrome）

Posterior ankle impingement (PAI) syndrome is one of the impingement syndromes involving the ankle. It is classically described in ballet dancers.

后踝撞击综合征（PAI）是踝关节撞击综合征的中的一种；常见于芭蕾舞演员。

Pathophysiology病理生理学

Thought to occur from repetitive ankle plantar flexion.

通常是由于踝关节反复跖屈引起。

Predisposing anatomical factors解剖学诱因

• Stieda process距骨后突

• os trigonum三角骨

• fracture involving lateral tubercle of posterior process of talus 累及距骨后突结节的骨折

• prominent superior surface of calcaneal tuberosity跟骨结节过度突出

• prominent down slope of posterior tibial articular surface胫骨后缘过度向下倾斜

• any abnormal calcification/ossification posterior to the ankle后踝处任何异常的钙化/骨化

     后踝撞击综合征除了骨性撞击外，后踝还包括软组织撞击：软组织因素造成的撞击较骨性因素多见。主要有以下几种：

①踇长屈肌腱鞘炎；

②胫距和距下关节炎而造成的滑膜增生；

③后踝间韧带(IML)嵌入；

④三角韧带后部(距胫后韧带)深层损伤后韧带纤维及其形成的肉芽或瘢痕组织嵌入内侧距骨和胫骨间关节隙，形成撞击；

⑤距腓后韧带及下胫腓后韧带纤维及其形成的肉芽组织或瘢痕组织嵌入外侧距骨和胫骨间关节隙，形成撞击。
