日间手术
R1. 在所有接受小型日间手术的患者中(例如包皮环切术、水肺切除术和输精管切除术),专家小组建议不要使用药物预防(强有力的、中等质量的证据),也不要使用机械预防(强有力的、中等质量的证据)。
注:专家小组认为,这些患者与普通人群的VTE风险、增加出血风险相近。
开放性根治性膀胱切除手术

机器人根治性前列腺切除术

非肿瘤泌尿手术

证据的总结
早期的主要指南处理手术中抗血栓药物的围手术期管理[2,33-35]优于最近的主要研究,包括大型、严格的随机试验[15,36-38]。关于抗血小板药物,最近的一项大型、严格的随机试验将阿司匹林与安慰剂进行了比较,结果表明阿司匹林增加了术后出血,但没有减少动脉血栓形成[15]。这些结果为阿司匹林以外的抗血小板药物提供了间接证据。虽然缺乏大型、严格的安慰剂对照试验来提供其他抗血小板药物的建议是有限制的,但考虑到类似的抗血栓和出血情况,间接证据为我们的建议提供了有用的信息。
在近期高质量证据出现之前提出的建议通常在围手术期建议患者定期使用替代抗凝血药物[39]。最近的证据表明,桥接增加出血,但不能预防血栓形成。因此,对于定期接受抗血栓药物治疗并考虑手术的患者,专家小组基本上有两项建议:
1)在手术前后的一段时间内停止抗血栓药物治疗;
2)对于那些血栓形成风险很高的患者,推迟手术直到风险降低。如果不能延迟,建议继续进行抗血栓治疗或在手术期间桥接。
手术前停止使用抗血小板药物5天是合适的时间,而停止使用抗凝血药物的最佳时间因抗凝血药物的不同而不同(详情请参见图2)。
R24. 在所有接受抗血小板药物(阿司匹林、氯吡格雷、普拉格雷、替卡格雷)治疗的患者中,除了血栓形成风险非常高的患者(见建议26和27),专家小组建议在手术前停用抗血小板药物,不要启动任何替代的抗血栓治疗(强有力的、高质量的证据)。
R25. 在手术前停用抗血小板药物的患者中,专家小组建议在出血不再是严重风险时(通常是手术后4天)重新开始使用抗血小板药物,而不是更长时间不使用(强有力的、中等质量的证据)。
R26. 在血栓形成风险极高的患者中接受抗血小板药物治疗(那些在6个月内放置药物洗脱支架的患者:6周内放置裸金属支架:专家小组建议延迟手术(强有力的、高质量的证据)的短暂性缺血发作(TIA)或30天内中风。
R27. 在血栓形成风险极高的患者中接受抗血小板药物治疗(那些在6个月内放置药物洗脱支架的患者:6周内放置裸金属支架:如果不能推迟手术,专家小组建议通过手术继续用药(弱,低质量证据)。
R28. 在所有患者接受抗凝药物(依诺肝素、低分子量肝素、华法林fondaparinux, dabigatran, apixaban, rivaroxaban, edoxaban),除非血栓形成的风险非常高(见建议26),专家组建议手术前停止药物 (见图2),不启动任何替代抗凝治疗(强,高质量的证据)。
注意:肌酐清除率< 30ml /min的患者不应使用达比加群、阿哌沙班、利伐沙班或依多沙班。
R29. 对于术前停用抗凝药物的患者,专家小组建议在出血不再是严重风险时(通常是术后4天)重新开始使用抗凝药物,而不是更长时间不使用抗凝药物(强有力的、中等质量的证据)。
R30. 对于新VTE患者,建议手术至少推迟一个月,如果可能的话,三个月,以允许术前停用抗凝血,而不是在血栓形成后一个月内进行手术(强有力的、高质量的证据)。
R31. 在接受抗凝血药物治疗并伴有严重血栓性血友病(如抗凝血酶缺乏和抗磷脂抗体综合征)的患者中,专家小组建议手术期间使用肝素或低分子肝素抗凝血,而不是在术前和术后停止抗凝血(弱的、低质量的证据)。
R32. 对于接受华法林治疗的高风险机械瓣膜(如笼球阀)患者,专家小组建议在术前和术后使用低分子肝素桥接,而不是在围手术期停止抗凝治疗(强有力的、高质量的证据)。
这些患者的抗凝包括在术前5天停用华法林,4天前开始使用低分子肝素,手术当天停用低分子肝素,术后重新使用低分子肝素和华法林。
研究建议
该指南的证据基础是有限的。很多关于基线风险的证据都很低或者质量很低[8,9]。前瞻性观察研究,以建立基础风险静脉血栓栓塞和出血的各种泌尿外科手术,以及解决患者血栓形成和出血的风险因素,将有必要创建更明确的指南。证据基础特别有限的程序包括机器人膀胱切除术、腹腔镜根治性肾切除术、开放肾输尿管切除术、TURP和脱垂手术。
要确定特定手术的VTE和出血的基线风险,需要符合特定方法学标准的研究,例如对患者群体的综合特征和随访时间的全面描述,使用的预防措施的文档,以及明确的标准,以证明DVT、PE和出血评估文档判断的可重复性。此外,血栓预防的最佳时机和时间仍不清楚。时间和持续时间问题最好通过大规模随机试验来解决。
